In a 2026 randomised trial of 119 adults with OCD, a concentrated four-day exposure programme reduced symptoms far more than an eight-week unguided online CBT course, and 61% of those treated had responded at three months compared with 25% [1]. The comparison was with a self-guided online course, not with weekly therapy, so it shows that concentrated exposure works well, not that four days is enough for everyone.

Can OCD Be Treated in Four Days at a glance

  • Trial: 119 adults with OCD, randomised to the Bergen 4-day treatment (B4DT) or eight weeks of unguided online CBT [1].
  • Result: at three months, 61.4% responded to B4DT against 25.0% for online CBT, and 42.1% were in remission against 13.6% [1].
  • Not tested: B4DT was not compared with weekly therapist-led exposure therapy, and people with schizophrenia, bipolar disorder, acute suicidality or substance dependence were excluded [1].
  • Other approaches: metacognitive therapy also beat treatment as usual in a 76-patient trial in Pakistan [2].
  • Apps: in a smartphone CBT trial, even low app users improved, though the group was small [3].

What did the four-day OCD trial test?

The Bergen 4-day treatment is a concentrated form of exposure therapy. Instead of one session a week for months, you work with a therapist for several hours a day over four consecutive days, after some preparatory sessions. In this trial from Germany, 119 adults aged 18 to 72 were randomly assigned to it or to an eight-week online CBT programme without a therapist. Raters who did not know which treatment people had assessed symptoms [1].

How well did it work?

OCD severity was measured with the Yale-Brown Obsessive Compulsive Scale (Y-BOCS). Right after treatment, the four-day group scored about 7.9 points lower than the online CBT group, a large difference. At three months it was still 5.6 points lower [1]. At that point, 35 of 57 people (61.4%) in the four-day group had responded, compared with 11 of 44 (25.0%) with online CBT, and 42.1% versus 13.6% were in remission [1].

Those are strong numbers. The follow-up was only three months, and some participants were missing from the follow-up counts, so longer-term trials are still needed.

Is four days really enough?

It depends on what you compare it with. The comparison group did unguided online CBT, with no therapist beside them. The trial did not test B4DT against ordinary therapist-led weekly exposure sessions [1]. It also excluded people with schizophrenia, bipolar disorder, acute suicidality or substance dependence, and people who had recently changed psychiatric medication [1]. If you have any of these, a longer, more individual plan is usually more appropriate.

What is exposure and response prevention?

Exposure and response prevention (ERP) is the core of most evidence-based OCD therapy. You are guided to approach what triggers your obsession, such as touching a door handle or leaving the stove alone, and to resist the ritual that usually follows. The anxiety rises and then falls without the ritual, which weakens the link between trigger and compulsion. It feels hard at first, which is why a trained therapist matters.

Are there other psychological options?

Yes. A 2026 trial in Pakistani outpatient services randomised 76 adults to 12 weekly sessions of metacognitive therapy or to treatment as usual. The therapy group had larger drops in Y-BOCS severity, by 4.0 points after treatment and 6.9 points at six months [2]. Metacognitive therapy works on beliefs about thoughts, such as the idea that having a thought means something terrible about you. The study could not confirm that changes in those beliefs came first and drove the improvement [2]. It matters because it was run in a South Asian setting, not only in Western clinics.

Do OCD apps help?

A smartphone CBT programme was tested over 12 weeks in 60 adults with OCD. About 35% used the app heavily, 52% moderately and 13% lightly. All three groups had significant symptom reductions, and the change in severity did not differ significantly between them [3]. Apps may help people who cannot reach a clinic, but this was a secondary analysis of a small trial, so it does not show an app replaces a therapist.

When should you seek help for OCD?

Seek help when rituals or intrusive thoughts take up an hour or more a day, make you late, hurt your work or studies, or strain your family. Family members often get pulled into rituals, so it helps to tell your clinician how home life is affected. If you ever have thoughts of harming yourself, call Tele-MANAS on 14416 (free, 24/7) or emergency services on 112.

Frequently asked questions

Can OCD be cured in four days?

No treatment can promise a cure. In one 2026 trial, 61% of adults who had four days of concentrated exposure therapy had responded at three months, and 42% were in remission [1]. Many people need continued practice and follow-up.

Is online CBT enough for OCD?

Unguided online CBT helped fewer people than concentrated therapist-led exposure in that trial [1]. Guided programmes and apps may suit mild cases or people who live far from a clinic.

What is the Y-BOCS?

It is a standard clinician-rated scale of OCD severity that covers time spent, distress and interference. Trials use the change in Y-BOCS score to judge whether a treatment works [1,2].

Who should not try an intensive four-day programme?

The trial excluded people with schizophrenia, bipolar disorder, acute suicidality or substance dependence [1]. Your clinician will check whether an intensive format is safe for you.

Is metacognitive therapy available for OCD?

It has shown benefit in a trial of 76 adults [2], but ERP remains the first-line psychological treatment [2]. Ask your therapist which approach fits your symptoms.
