Can Weight-Loss Injections Treat Sleep Apnoea?

28 Sept 2026

Нейронауки и больSleep ApnoeaGLP-1

In a 2026 meta-analysis of six trials, GLP-1-type weight-loss injections lowered breathing interruptions in moderate-to-severe sleep apnoea by about 10 events an hour, alongside roughly 10 kg of weight loss [1]. That is a real benefit, but the authors state the findings do not establish these drugs as a substitute for CPAP [1]. For most people they are an addition to proper diagnosis and treatment, not a replacement.

Can Weight-Loss Injections Treat Sleep Apnoea at a glance

  • What was studied: 6 randomised trials, 1,069 adults with moderate-to-severe sleep apnoea, using liraglutide or tirzepatide [1].
  • Main result: fewer breathing interruptions per hour (AHI down by about 10) and about 9.7 kg lower body weight [1].
  • Caveat: results varied a lot between trials, so the exact size of benefit is uncertain [1].
  • Not shown: oxygen levels during sleep did not change significantly, and the drugs were not tested as a replacement for CPAP [1].
  • Other options: a custom mandibular advancement device performed close to CPAP in a small study of people who could not tolerate CPAP [2].

What did the new study find?

The review pooled six randomised trials. Four tested liraglutide and two tested tirzepatide, in 1,069 adults with moderate-to-severe obstructive sleep apnoea [1]. Compared with control groups, the injections reduced the apnoea-hypopnoea index (AHI), the number of breathing interruptions per hour, by about 10 events. Body weight fell by about 9.7 kg, and systolic blood pressure by about 4.8 mmHg [1].

Two things did not improve significantly: the oxygen desaturation index and the lowest oxygen saturation overnight [1]. Rates of adverse events were not higher than in control groups in this analysis [1].

Does this mean you can stop CPAP?

No. The authors say clearly that the findings do not make these drugs a substitute for CPAP [1]. An AHI that drops by 10 is helpful, but someone starting at 40 or 50 events an hour may still have significant apnoea afterwards. The trials also varied widely in their results, which the authors flag as a limit on how far the pooled number can be trusted [1].

If you have been prescribed CPAP, keep using it until your doctor has repeated your sleep study and decided otherwise.

Who might benefit from a weight-loss drug?

The evidence applies to adults who have both obstructive sleep apnoea and excess weight, since the trials enrolled people with obesity [1]. Weight reduction is already recommended for overweight patients with sleep apnoea [1]. A drug is one way to get there, alongside food changes and exercise.

It does not apply to everyone. Sleep apnoea in a slim person is often about jaw structure or airway anatomy, and weight loss will not fix that. Whether a GLP-1 drug is suitable for you is a decision for your doctor, because these medicines carry their own risks and are not appropriate for everyone.

What if CPAP does not suit you?

Many people stop using CPAP because of the mask, the noise or the pressure. In a small crossover study of 20 adults who had not stuck with CPAP, a custom digital mandibular advancement device reduced the oxygen desaturation index from about 13.6 to 6.1 events an hour. CPAP brought it to 5.3, and the difference between the two was not statistically significant [2]. CPAP did give slightly better oxygen saturation, though the gap was small [2].

Twenty people is a small sample, so treat this as a signal, not a verdict. It does suggest that a person who cannot tolerate CPAP has options worth discussing.

Can exercise help even without weight loss?

A small randomised trial of 25 adults with moderate-to-severe sleep apnoea tested eight weeks of resistance training three times a week. Sleep quality scores and quality of life improved compared with the control group, though daytime sleepiness scores did not [3]. The trial was small and measured how people felt, not their breathing events, so it supports exercise as a useful extra rather than a treatment on its own [3].

What should you do if you snore heavily or feel tired all day?

Loud snoring, gasping at night, morning headaches and daytime sleepiness are reasons to get tested, not to try a drug first. Sleep apnoea is diagnosed with a sleep study. The result tells you how severe it is, and that decides which treatment makes sense: CPAP, a mouth device, weight management, or a combination.

Getting help in Delhi

At Brain Bridge Clinic in Safdarjung Enclave, New Delhi, a sleep assessment starts with your symptoms, a bed partner's account of your snoring or pauses, and your medical history. Where it is appropriate, an overnight sleep study is arranged to confirm whether you have apnoea and how severe it is. Dr. Dhiraj Saxena, our pulmonologist, can then talk through the options with you, including CPAP titration and how weight management fits in. If you or someone at home is worried about your sleep, you are welcome to book an assessment.

Frequently asked questions

Can weight-loss injections cure sleep apnoea?

No. In the 2026 meta-analysis they reduced breathing interruptions by about 10 an hour on average, but the authors say this does not establish them as a substitute for CPAP [1]. Some people will still need ongoing treatment.

Do the injections work if I am not overweight?

The trials enrolled people with obesity, so the results do not tell us whether the drugs help leaner people with sleep apnoea [1]. Apnoea in a person of normal weight usually needs a different explanation and approach.

Is a mouth device as good as CPAP?

In one small crossover study of 20 adults, a custom mandibular device and CPAP gave similar results on oxygen desaturation and snoring [2]. Larger trials are needed, and a dentist or sleep physician must fit the device.

Are these injections safe?

The pooled analysis did not find more adverse events than in control groups [1]. That does not mean they suit everyone, so they should be prescribed and monitored by a doctor.

How do I know if I have sleep apnoea?

A sleep study is the way to confirm it. Persistent loud snoring, witnessed pauses in breathing and daytime tiredness are reasons to ask for one.
